Appointment Request

The first step towards a beautiful, healthy smile is to schedule an appointment.  If you are interested in scheduling an appointment, please feel free to contact our office by phone or by simply completing an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to login and access our new patient forms.  These forms will need to be completed prior to your arrival.  Completing the forms will expedite the process and allow us to attend to your dental needs promptly.  If you have any questions, concerns or need assistance in completing the forms, please feel free to contact our office and we will be more than happy to assist you.  

Dental Insurance

We accept all PPO dental insurance plans and file insurance claims as a convenience to you. 

Dental insurance benefits are different from medical insurance benefits.  Dental insurance typically does not cover all services.  Some policies cover only basic services and have annual limits on benefit payments.

Dr. Oefinger will prescribe treatment based on your oral health condition, not your insurance benefits. 

Please feel free to contact our office if your treatment plan requires more comprehensive dental work and would like to discuss financial arrangements. 

Payment Options

Payment is expected at the time services are performed.  For your convenience, we accept cash, personal checks, money orders, and most major credit cards. 

Even if you have dental insurance, you are likely to have some out-of-pocket expenses associated with your dental care.  Through our partnership with CareCredit, we offer flexible payment plans to qualified applicants.
